Organizational Diagnostic Models: A Comprehensive Review

Introduction
Organizational diagnosis refers to the systematic assessment of an organization’s current state to understand problems, pinpoint root causes, and identify improvement opportunities. Since the emergence of Organization Development (OD) in the mid-20th century, many diagnostic models have been proposed to guide this process. Early OD focused on human behavior, but over time diagnosis has become more strategic and holistic[1][2]. Today’s models typically adopt an open-systems perspective, examining interactions between people, structure, and the external environment (building on Katz & Kahn’s open-systems theory)[2].
This review scans leading academic literature – with emphasis on Bushe and Marshak’s perspectives – to identify up to 20 of the most influential organizational diagnosis models (including classic frameworks). For each model, we note its core concept, origin, and quality (e.g. publication in scholarly outlets or uptake by consulting firms). We then propose criteria for classifying these models and key questions that guide consultants in choosing the most appropriate model for a given situation. Finally, we discuss fundamental assumptions, advantages, and disadvantages of using diagnostic models in practice, to ensure their effective application in professional OD diagnosis.

Key Organizational Diagnosis Models
(Classics and Leading Frameworks)
Below we list major organizational diagnostic models in roughly chronological order. These models have shaped the field and are widely recognized in OD literature and practice:
Lewin’s Force Field Analysis (1951): Kurt Lewin’s model views any situation as a balance of driving forces and restraining forces. Organizational change is achieved by strengthening driving forces or reducing restraining forces[3]. Lewin introduced this simple yet powerful tool for analyzing change dynamics in his seminal work Field Theory in Social Science (Lewin, 1951). APA Reference: Lewin, K. (1951). Leavitt’s Diamond Model (1965): Harold J. Leavitt proposed that an organization can be diagnosed via four interactive variables: Task, Structure, Technology, and People. A change in one area affects the others[4]. This “diamond” model (also called Leavitt’s model) highlights internal alignment but originally did not explicitly include the environment[5]. Likert’s System Analysis (1967): Rensis Likert developed a model of organizational systems ranging from System 1 (exploitative-authoritative) to System 4 (participative-group). He identified key organizational dimensions (leadership, motivation, communication, decisions, goals, control) and profiled organizations on these scales[6]. Likert’s model is somewhat normative, asserting that System 4 (high participation) is most effective. It was detailed in The Human Organization: Its Management and Value (Likert, 1967). Open Systems Model (1960s–1970s): The open-systems approach (Katz & Kahn, 1978; also Emery & Trist) is a theoretical foundation rather than a single model. It frames organizations as open systems that take inputs from the environment, transform them, and produce outputs, with feedback loops for continuous adaptation[2]. All modern diagnosis models build on open-systems theory[2], stressing the interdependence of subsystems and environment. Many OD texts (e.g. Cummings & Worley, 2009) present an open-systems diagnostic schema (inputs → design components → outputs) as a baseline model. APA Reference: Katz, D., & Kahn, R. L. (1978). Weisbord’s Six-Box Model (1976): Marvin Weisbord introduced a practical framework to “look for trouble” in six key areas: Purpose, Structure, Rewards, Relationships, Helpful Mechanisms, and Leadership[7][8]. The model is often depicted as six boxes (with Leadership at the center, as it connects to all other boxes)[9]. Weisbord suggested diagnosing “gaps” in each box: e.g. gaps between what exists vs. what should be, between formal and actual practice, or between different organizational units[10][11]. The Six-Box model is simple and widely used by practitioners because it’s easy for clients to understand[12]. However, its simplicity is also a drawback – it lacks a detailed theoretical basis to determine which gaps are most critical, and it doesn’t guide how to fix them[13]. Nadler-Tushman Congruence Model (1977/1980): David Nadler and Michael Tushman developed the Congruence Model to diagnose organizational performance by examining the alignment (congruence) between key elements: Environment, Resources, History (inputs); Strategy; Work, People, Structure, Culture (components); and Outputs at the individual, group, and organizational level[14]. The central idea is that the better these elements fit together, the higher the performance[14]. Misalignments (incongruences) pinpoint trouble spots. This comprehensive model was originally presented in the late 1970s and published in Organizational Dynamics (Nadler & Tushman, 1980). McKinsey 7-S Framework (1980–1982): The 7-S model, developed by McKinsey consultants (Waterman, Peters & Phillips, 1980), examines an organization through seven interrelated elements: Strategy, Structure, Systems, Shared Values, Skills, Style, Staff[15][16]. The first three are “hard” factors, and the latter four are “soft” factors; Shared Values (vision/culture) is at the center as it guides all others[17][18]. The 7-S model’s strength is in showing that organizational effectiveness emerges from the alignment of all seven elements[19]. It became widely popular in the early 1980s through business books (In Search of Excellence, 1982) and is still used by consulting firms for a broad organizational review. Galbraith’s Star Model (1982): Jay Galbraith’s Star Model is an organization design framework for diagnosis and design alignment. The star has five points representing Strategy, Structure, Processes, Rewards, and People practices[14]. Strategy drives the other components; for an organization to perform well, all points must be aligned with the strategy. Galbraith first discussed this model in the early 1980s (Galbraith, 1982) and later in his book Designing Organizations (Galbraith, 1995). Tichy’s Technical Political Cultural (TPC) Framework (1983): Noel Tichy presented a comprehensive model highlighting that effective change involves managing three intertwined dynamics – the Technical, Political, and Cultural systems of the organization[20]. Tichy’s framework includes Inputs (External Environment & Organizational History, plus Resources) and Throughputs (the “change levers”: Mission/Strategy, Tasks, People, Organizational Processes) and Outputs (Organizational Performance/Effectiveness)[21][22]. What’s unique is the “TPC overlay”: across all these elements, one must consider technical aspects (workflow, tools), political aspects (power, conflict), and cultural aspects (values, norms)[23]. Tichy uses a “rope” metaphor: the technical, political, and cultural strands are woven together – they must be managed in tandem for change to succeed[24]. This model emphasizes that organizational diagnosis is complex and multifaceted[25]. It appeared in Tichy’s book Managing Strategic Change: Technical, Political, and Cultural Dynamics (1983). Nelson & Burns’s High-Performance Programming (1984): D. Nelson and F. Burns proposed a model that categorizes organizations into four developmental levels of increasing effectiveness: Reactive (Level 1), Responsive (Level 2), Proactive (Level 3), and High Performing (Level 4)[26]. Each level reflects a different time orientation, management style, structure, and culture – higher levels exhibit longer-term focus, empowerment, teamwork, and adaptability[27][28]. The model’s purpose is to diagnose the organization’s current level and guide cultural interventions to reach higher performance (e.g. moving from reactive to proactive)[29]. Nelson and Burns’ framework is prescriptive (like Likert’s, it implies the “high-performing” state is the goal)[30]. Harrison’s Diagnosing Individual and Group Behavior (1987): Michael Harrison presented a model focusing on internal group and individual-level outputs as indicators of organizational health. In Diagnosing Organizations (1987), he introduced a framework examining how individual, group, inter-group, and organizational factors (like tasks, structure, interpersonal relations, etc.) produce outcomes such as job satisfaction, performance, and quality of work life[31]. The model includes multiple feedback loops and interrelations, recognizing that many factors influence group effectiveness (some influences are one-way, others reciprocal)[32]. This approach was later refined with Arie Shirom (Harrison & Shirom, 1999) into a “sharp-image” diagnosis approach, emphasizing targeted analysis of specific problem areas. Burke–Litwin Model of Organizational Performance and Change (1992): W. Warner Burke and George Litwin developed a causal model linking the external environment to internal factors and business results. The Burke–Litwin model specifies 12 key dimensions organized by level: External Environment influences the transformational factors (Leadership, Mission & Strategy, Organizational Culture), which in turn affect transactional factors (Structure, Management Practices, Systems (policies/procedures), Climate, and individual-level factors like Motivation, Skills/Job fit) – all ultimately impacting Overall Performance[33]. A hallmark of this model is the distinction between transformational change vs. transactional change and the causal relationships hypothesized between elements[34]. For example, external changes primarily impact leadership, strategy, and culture, which cascade down to affect other components[34]. The model can be used to diagnose where a change initiative should focus and predict effects of changes[33]. Burke & Litwin published this framework in the Journal of Management (a top-tier journal)[35], lending it high academic credibility. Competing Values Framework – CVF (Quinn & Rohrbaugh, 1983; Cameron & Quinn, 1999): The Competing Values Framework emerged from research on organizational effectiveness criteria. It maps organizational culture into four quadrants based on two axes (flexibility vs. control, internal vs. external focus): Clan, Adhocracy, Market, and Hierarchy cultures[36][37]. Each represents competing values (e.g. Clan emphasizes participation, Hierarchy emphasizes stability). Cameron & Quinn’s popular book Diagnosing and Changing Organizational Culture (1999) built on CVF to provide the OCAI (Organizational Culture Assessment Instrument) for diagnosing an organization’s current and desired culture type. The CVF is academically well-established (appearing in Strategic Management Journal and others) and practically used by consultants to diagnose culture and leadership styles. Schein’s Cultural Model (1985): Edgar Schein’s model isn’t a step-by-step diagnostic tool, but it provides a theory for diagnosing organizational culture at three levels: Artifacts (visible structures and processes), Espoused Values, and Basic Underlying Assumptions (unconscious, taken-for-granted beliefs). Schein (1985, Organizational Culture and Leadership) argued that diagnosing culture requires probing these levels (e.g. through interviews and observations) to reveal deep assumptions. This model guides consultants in cultural diagnosis by reminding them that surface symptoms often trace to hidden fundamental assumptions. APA Reference: Schein, E. H. (1985). Bolman & Deal’s Four Frames Model (1991): Lee Bolman and Terrence Deal proposed that organizational situations can be viewed through four “frames” or lenses – Structural, Human Resource, Political, and Symbolic[38]. Using multiple frames in diagnosis allows a consultant to uncover different aspects of problems (e.g. a decision issue might be seen as a structural design flaw, a conflict of interests, a HR issue, or a cultural symbolism issue). Their book Reframing Organizations (1991) popularized this approach. While not a traditional diagnostic checklist, the Four Frames model is widely taught for broadening consultants’ diagnostic perspective and avoiding one-dimensional analysis. Socio-technical Systems (STS) Diagnosis (1950s–1980s): Originating from Eric Trist and colleagues at the Tavistock Institute, STS theory posits that organizations consist of social and technical subsystems that must be jointly optimized. An STS diagnosis examines work system design (technology, workflows) and team dynamics (people, culture) to find misalignments. For example, a consultant might diagnose that a highly automated technical process is at odds with a traditional bureaucratic social structure, causing inefficiency. Though STS is a foundational OD approach rather than one codified model, its principles underlie many later diagnostics (e.g. congruence models). APA Reference: Trist, E., & Emery, F. (1960). European Foundation for Quality Management (EFQM) Excellence Model (1991): The EFQM model is a self-assessment framework widely used in Europe to diagnose organizational excellence. It outlines nine criteria (five Enablers: Leadership, Strategy, People, Partnerships & Resources, Processes; and four Results: People Results, Customer Results, Society Results, Business Results). Organizations are scored on each, identifying strengths and areas for improvement. While developed for quality award assessments, EFQM has been adopted by consulting firms for organizational diagnostics beyond just quality – providing a structured, criteria-based evaluation. Balanced Scorecard (Kaplan & Norton, 1992): The Balanced Scorecard is primarily a strategic performance management tool, but it serves as a diagnostic framework by examining an organization from four perspectives: Financial, Customer, Internal Processes, Learning & Growth. By assessing metrics and objectives in each perspective, consultants diagnose imbalances or misalignments in strategy execution. Its widespread adoption (by ~70% of Fortune 500 companies by the 2000s) and consulting use (Bain & Co ranks it among top management tools) makes it a relevant model when diagnosing strategic performance issues. Denison’s Organizational Culture & Effectiveness Model (1990): Daniel Denison’s model links culture traits to performance. It measures four cultural traits – Involvement, Consistency, Adaptability, Mission – each split into three sub-dimensions. It’s typically visualized as a circle (each quadrant a trait). The model comes with a validated survey (Denison Organizational Culture Survey) to diagnose an organization’s cultural strengths and weaknesses and predict performance outcomes like ROI or quality. Falletta’s Organizational Intelligence Model (2008): Salvatore Falletta proposed the Organizational Intelligence Model as a modern synthesis. It integrates elements from earlier models (strategy, structure, culture, etc.) and emphasizes measurement via surveys. Falletta’s model was accompanied by an “Organizational Intelligence Survey” and was described in a 2008 HR Intelligence Report[39][3]. (The above list is not exhaustive – numerous other models and variations exist. For instance, Sharp-Image Diagnosis (Harrison & Shirom, 1999) advocates crafting a tailored model for each case rather than using a pre-set model[40][41]. Additionally, specialized diagnostics focus on particular domains, like process mapping, stakeholder analysis, or change readiness. However, the models listed represent the most influential and widely cited frameworks in organizational diagnosis.)

Criteria for Classifying and Selecting Diagnostic Models
Given the many models, OD practitioners should thoughtfully classify and choose a model suited to the situation. Key criteria and guiding questions include:
Scope and Level of Analysis: Does the model address the whole organization or a specific level (individual, group, or subsystem)? For example, Burke–Litwin and 7-S are enterprise-wide, whereas Harrison’s model drills down to group dynamics[31]. Question: “Do I need a high-level systems view or a deep dive into team-level issues?”
Comprehensiveness vs. Focus: Some models (e.g. Burke–Litwin) cover a broad array of variables (strategy, culture, systems, etc.), whereas others focus on a few key factors (e.g. Weisbord’s six factors or Likert’s leadership/communication factors). A broad model yields a holistic diagnosis but requires more data; a focused model is simpler but may overlook something. Question: “Is a quick, focused check-up sufficient, or do we suspect multifaceted problems needing a comprehensive scan?”
Open-System Inclusion of Environment: Models vary in how explicitly they consider the external environment. Open-systems models (e.g. Nadler-Tushman, Burke–Litwin) include external inputs and feedback[21][34], whereas simpler ones (like Leavitt’s or early 7-S) originally treated the organization in isolation[5]. Question: “How critical are external factors (market, technology, regulations) to the issues at hand?” (If very critical, choose a model that brings in the environment.)
Descriptive vs. Normative Orientation: Descriptive models map out “what is” and help envision “what could be” without prescribing one “best” configuration[42]. Examples: Congruence model, 7-S, Four Frames – they help uncover misalignments but don’t dictate the ideal state. Normative models embed an ideal: e.g. Likert’s System 4 or Blake & Mouton’s Grid (which prescribes high concern for both people and production as best)[43]. Normative models can guide toward a known ideal, but may not fit every context. Question: “Does this situation call for measuring against a universal benchmark of excellence (normative), or simply understanding unique dynamics (descriptive)?”
Quantitative vs. Qualitative Approach: Some frameworks come with quantitative diagnostic instruments (surveys, metrics) – e.g. Denison’s survey, Competing Values OCAI, or any model that can be turned into a questionnaire (many companies have surveys for Burke–Litwin variables, etc.). Others are more qualitative, relying on interviews, observations, and facilitator judgment (e.g. Weisbord’s model often uses workshops to identify gaps). Question: “Do we have the time and resources for a survey and statistical analysis, or will interviews and workshops suffice?” Also, consider client culture: data-driven organizations may favor a model with metrics.
Theoretical Rigor and Evidence Base: From an academic standpoint, models published in high-quality journals or validated by research (e.g. Burke–Litwin in Journal of Management[35], or research validating CVF and Denison’s model) have strong credibility. Others stem from consulting experience or books (e.g. 7-S from In Search of Excellence). While both can be useful, the choice might depend on the client’s preference for evidence-based practices. Question: “Does the client (or the OD team) value a scientifically validated model, or will a practitioner-proven framework be equally accepted?”
Consulting Firm Preference and Popularity: Leading consulting firms often have preferred frameworks (sometimes proprietary variants). For instance, McKinsey consultants routinely use 7-S for organizational alignment checks; culture consultancies might use Cameron & Quinn’s CVF or Denison’s model; human performance improvement firms might favor Gilbert’s behavior engineering model (if we included HR-focused diagnostics). If the organization has prior experience with a model (e.g. already did a Balanced Scorecard or an EFQM assessment), continuity might be a factor. Question: “Which models are respected in this organization or industry? (E.g. manufacturing firms might be familiar with EFQM or Lean diagnostics; tech startups might resonate with learning organization or agile assessments.)”
Diagnostic Purpose/Problem Area: Perhaps most importantly, the nature of the presenting problem guides model choice. Different models illuminate different issues:
If the issue is overall performance decline with unclear cause – a comprehensive model like Burke–Litwin or Nadler-Tushman can systematically pinpoint misalignments.
If the issue is cultural or merger-related – culture models (Schein, CVF, Denison) are appropriate.
For strategy implementation problems – 7-S or Balanced Scorecard to check alignment of strategy with structure, systems, etc.
For team or interpersonal conflict – a group-level model (Harrison’s, or even something like Likert’s profile focusing on leadership climate).
For change readiness – a force-field analysis or a readiness assessment model (Armenakis et al. have one) might be useful.
Question: “What type of problem are we diagnosing – cultural, structural, strategic, human, or a bit of everything?” Matching the model focus to the problem ensures relevant data is collected.
Client Involvement and Culture: Simpler models like Six-Box are easily explained and can involve managers in identifying gaps (which can build buy-in). More complex models (TPC or large surveys) might require expert analysis that could alienate or overwhelm stakeholders. Also, organizational culture matters – an open, participative culture might readily engage in an Appreciative Inquiry (a dialogic diagnostic approach focusing on strengths), whereas a very analytical culture might prefer hard data and detailed models. Question: “Which diagnostic approach will the client engage with and trust? Does the client prefer broad participation (then choose an easy, participative model) or an expert study (then a rigorous analytical model can work)?”
In practice, experienced consultants often combine models or adapt them. For instance, one might use open-systems as a meta-framework, but zoom in with a specialized model for a particular box (e.g. use Weisbord’s six boxes to structure data gathering, but within the “culture” box, apply Schein’s levels to dig deeper). Harrison & Shirom (1999) advocate a contingent approach: start with a broad scan, then develop a custom model focusing on the core problems in that organization[40][41]. The custom model can draw on elements of known models and the consultant’s own experience[41]. This “sharp-image” diagnosis ensures the model fits the context, rather than force-fitting the organization to a single pre-existing model[44].
Guiding Questions for Model Selection:
To summarize, a consultant might ask: - “What is the primary goal of this diagnosis – general check-up or specific problem analysis?” - “What level(s) of the organization need diagnosis?” (Whole organization vs. divisions vs. teams) - “Which model’s assumptions fit the client’s situation and mindset?” (e.g. stability vs. change, internal vs. external focus) - “What resources (time, data access, expertise) do we have?” (choose a model feasible to implement with available resources) - “How will we involve the client?” (choose a model that the client will find credible and actionable – e.g. a highly analytical model for an engineering firm, perhaps a more dialogic approach for a non-profit community org) - “What has worked in the past or in similar organizations?” (leverage known successes – if many firms in this sector use EFQM or similar, that may ease acceptance)
By considering these criteria, an OD practitioner can classify potential models and select the most appropriate diagnostic framework (or hybrid of frameworks) to guide a successful organizational diagnosis.
Key Assumptions, Principles, Advantages & Disadvantages of Diagnostic Models
When applying any organizational diagnostic model, it’s crucial to understand its underlying assumptions and principles, as well as its strengths and limitations. Being mindful of these factors helps ensure the diagnosis leads to effective action and avoids missteps. Below, we outline some common assumptions and pros/cons, highlighting what should guide their use in practice:
Open-System & Alignment Assumption: Virtually all modern models assume organizations are open systems – i.e. a change in one component will ripple through others, and organizations must adapt to their environment[2]. This holistic mindset is a key principle: look at patterns of alignment or misalignment. Advantage: Encourages comprehensive data gathering (avoids silo focus) and identifies root causes (e.g. low sales may trace to misaligned structure or culture, not just sales team skills). Disadvantage: The complexity can be overwhelming – diagnosing “everything relates to everything” might yield an unwieldy amount of data. Practitioners must balance thoroughness with the ability to prioritize issues.
Objectivist vs. Constructivist View: Traditional diagnostic models are built on a positivist, objectivist premise – the organization is a real system that can be objectively measured and understood by an expert diagnostician[45][46]. For example, a survey can pinpoint “the truth” about morale or communication frequency. Advantage: This allows use of scientific methods (surveys, benchmarks, causal analysis) and lends credibility when presenting findings (“the data show X cause of Y”). Disadvantage: It may overlook that organizations are also social constructions; people’s narratives and interpretations matter. Newer Dialogic OD approaches (Bushe & Marshak, 2009) argue that focusing only on “measuring the current state against a model” misses how multiple realities and conversations shape change[46][45]. Principle: Practitioners should be aware that diagnoses can become self-fulfilling labels – involving members in sense-making (not just data analysis) can create more buy-in for change.
Stability vs. Change Assumption: Diagnostic models often take a snapshot of the organization – implicitly assuming the organization has a discernible current state to be studied prior to intervention.
